  (Child must have either been born in formerly known Garden State Hospital located on Route 73, Marlton, NJ up until June 1987, or as a home birth occurring within the Township of Evesham)

• Death Certificate
  (The passing of decedent must have occurred in Township of Evesham)

• Domestic Partnership Certificate
  (Vital Record Copy)

• Marriage or Civil Union Certificate
  (Issued After a marriage or civil union ceremony which was performed in the Township of Evesham by New Jersey state-recognized Officiate)
